The Tests That Matter

What Standard Panels Miss — and Why It Matters

Here are the markers most commonly absent from routine bloodwork that carry the highest clinical value for prevention:

  • ApoB — counts every atherogenic lipoprotein particle. Patients with "normal" LDL can have dangerously elevated ApoB
  • Lipoprotein(a) — genetically determined, independent cardiovascular risk factor. Only needs checking once. Many patients have no idea it's elevated
  • Fasting Insulin — reveals insulin resistance a decade before fasting glucose becomes abnormal
  • HOMA-IR — calculated insulin resistance score. Optimal below 1.5
  • TG:HDL Ratio — one of the most reliable non-invasive markers of insulin resistance and cardiovascular risk
  • hsCRP — high-sensitivity C-reactive protein, a marker of systemic inflammation linked to cardiovascular and metabolic disease
  • Coronary Calcium CT (CAC) — imaging that directly measures arterial calcification. One of the most powerful cardiovascular risk predictors available

Going Further When It's Warranted

Advanced Imaging: CAC & DEXA

Bloodwork tells us a great deal — but sometimes the clearest answers come from imaging. When your clinical picture calls for it, I order and interpret two of the most useful, evidence-based tests in preventive medicine. These aren't ordered for everyone; they're used when they'll genuinely change your understanding or your plan.

Coronary Artery Calcium (CAC) Score

A CAC scan is a quick, low-dose CT of your heart that directly measures the calcified plaque in your coronary arteries. Instead of estimating your heart risk from cholesterol numbers, it shows what's actually there.

Why it's powerful:

  • One of the strongest available predictors of future heart attack — often better than cholesterol alone
  • A score of zero is genuinely reassuring and can help avoid unnecessary medication
  • A high score can justify treating aggressively before a first event — turning guesswork into a clear decision
  • Especially useful when you're "on the fence" about starting a statin or other therapy

Honest limitations:

  • Involves a small dose of radiation (low, but not zero)
  • Measures calcified plaque — very early soft plaque may not show yet, so a zero isn't a permanent guarantee
  • Not useful for everyone (for example, if you already have known coronary disease, it adds little)
  • It's an out-of-pocket cost, done at an imaging center near you

DEXA Body Composition Scan

Most people know DEXA for bone density — but the same scan precisely measures your body composition: lean muscle mass, fat mass, and, importantly, visceral fat (the metabolically dangerous fat around your organs) that the scale and BMI completely miss.

Why it's useful:

  • Visceral fat drives insulin resistance and cardiovascular risk — and you can carry a lot at a "normal" weight
  • Tracks real muscle mass, which matters for metabolism, strength, and healthy aging
  • An honest baseline — and a powerful way to measure real progress over time on a hormone, GLP-1, or metabolic plan (are you losing fat or losing muscle?)
  • Also gives you bone density as a bonus

Honest limitations:

  • An out-of-pocket cost, though widely available and affordable in most metro areas — often self-scheduled
  • Very low radiation, but again not zero
  • A snapshot — most valuable when repeated over time to track change, not as a one-off number

These are tools, not requirements. I recommend CAC or DEXA only when your clinical picture makes them genuinely worthwhile — never as a standard add-on for everyone. When they're warranted, I'll order them, help you find a convenient location, and interpret the results in the full context of your labs, history, and goals.
