By accessing or using the Beacon Hormone & Wellness website at beaconhormone.com, you agree to be bound by these Terms of Service. If you do not agree, please do not use this website.

These terms apply to the website only. Separate terms may apply to clinical services provided through our patient care platform, Practice Better, which are governed by our patient intake agreements.

About This Website

Beacon Hormone & Wellness is a Washington State telemedicine medical practice operated by Sergio Fernandez, FNP-C, a board-certified Family Nurse Practitioner licensed in Washington State.

This website is provided for informational and educational purposes. It is not a substitute for professional medical advice, diagnosis, or treatment.

Geographic Limitations

Clinical services provided by Beacon Hormone & Wellness are available to residents of Washington State only. Sergio Fernandez, FNP-C is licensed to practice in Washington State and may only provide clinical care to patients who are physically located in Washington State at the time of their telehealth appointment.

Use of this website from outside Washington State — or from outside the United States — does not create a patient-provider relationship and does not entitle any person to clinical services.

Patient Relationship

A formal patient-provider relationship with Beacon Hormone & Wellness is established only upon completion of intake paperwork, agreement to our patient care terms, and the scheduling of an initial consultation through our official booking platform.

Viewing this website, downloading educational materials, subscribing to our email list, or sending an email inquiry does not create a patient-provider relationship or any clinical obligation.

Booking & Cancellation Policy

Initial Consultation

• The initial consultation fee is $175 and is collected at the time of booking
• Cancellations or reschedules made at least 24 hours before the appointment are eligible for a full refund or reschedule at no charge
• Cancellations made with less than 24 hours notice or missed appointments without notice are non-refundable

Ongoing Membership

• Monthly membership is $135 per month, billed on a recurring basis
• Membership may be cancelled at any time with 30 days written notice to sergio@beaconhormone.com
• No refunds are provided for partial months already billed
• Beacon Hormone & Wellness reserves the right to terminate membership at its discretion with reasonable notice

Metabolic Reset Program

• The Metabolic Reset program fee is $497 and is collected at enrollment
• Refunds are available within 7 days of enrollment if no visits have been completed
• After the first visit has been completed, the program fee is non-refundable
• Lab costs, medications, and supplements are billed separately and are not included in the program fee

Scope of Practice

Beacon Hormone & Wellness provides care within the scope of a Family Nurse Practitioner licensed in Washington State, focusing on hormone health, metabolic wellness, and preventive medicine. Sergio Fernandez, FNP-C does not provide emergency care, acute illness management, or care outside the defined clinical focus of this practice.

Patients are encouraged to maintain a relationship with a primary care provider for acute and comprehensive care needs outside the scope of this practice.

Telehealth Requirements

All clinical visits are conducted via telehealth. Patients must:

• Be physically located in Washington State at the time of each visit
• Have access to a device with video and audio capability
• Use the Practice Better platform as directed for all clinical communication
• Maintain a current and accurate medical history on file

All content on this website — including text, design, graphics, logos, and educational materials — is the property of Beacon Hormone & Wellness and Sergio Fernandez, FNP-C. You may not reproduce, distribute, or republish any content from this website without prior written permission.

Educational materials downloaded for personal use (such as the Metabolic Health Starter Guide) are provided for individual, non-commercial use only.

Limitation of Liability

To the fullest extent permitted by law, Beacon Hormone & Wellness and Sergio Fernandez, FNP-C shall not be liable for any indirect, incidental, or consequential damages arising from your use of this website or reliance on its content. Our total liability for any claim arising from use of this website shall not exceed the amount you have paid to us in the 30 days preceding the claim.

Governing Law

These Terms of Service are governed by the laws of the State of Washington. Any disputes arising from these terms or your use of this website shall be resolved in the courts of Washington State.

Changes to These Terms

We reserve the right to update these Terms of Service at any time. Changes will be posted to this page with an updated date. Continued use of the website after changes are posted constitutes acceptance of the revised terms.
